Cancellation of Registration (cont)

• Cancellation may be done electronically or by mail and must include:

  • Facility's registration number
  • Whether facility is domestic or foreign
  • Facility name and address
  • Name and address of the individual submitting the cancellation
  • Statement certifying information is true and accurate and person is authorized to submit cancellation
